When you are admitted, you will receive an identification bracelet to be worn at all times. We use this bracelet to identify you before treatments, medications and in emergency situations. Please do not remove it until after you are discharged. If you are ever in doubt about your care, please let someone from your health-care team know.

What to bring:

• Your Ontario Health Card and private health insurance information;
• Medication, or if not available, a list of all the medications and amounts you are currently taking (including prescription drugs, over-the-counter drugs, vitamins, food supplements and herbal products);
• Appropriate, comfortable clothing and non-slip footwear;
• Toothbrush, toothpaste, unscented shampoo, body lotion, shaving supplies, etc.;
• Comb or brush;
• Sanitary items;
• Child’s favourite toy or blanket;
• Newborn sleepers and car seat for discharge.
