Geriatric Medicine

Clinical
The Division of Geriatrics supports the South Eastern Regional Geriatrics Program (SERGP), which is a "Priority Program of Ministry of Health and Long-Term Care's. Together they offer a full spectrum of specialty geriatric assessments for patients in Southeastern Ontario. The target population consists of those individuals generally over age 65 that, as a result of multiple complex health problems, require comprehensive, multidisciplinary, and multidimensional geriatric assessment, treatment, and rehabilitation.

Community outreach assessments are conducted in patients' homes, long-term care facilities, and non-teaching hospitals in Southeastern Ontario. Individuals seen through the outreach service are often triaged to other components of specialized geriatric services. Tele-consultation and educational mentoring roles have also been supported as part of a strategy of increasing knowledge and awareness within community health centres within both HPE and LLG counties.

The Division provides regular outpatient clinics at Providence Continuing Care Centre St. Marys of the Lake Hospital (PCCC) and outreach clinics in Brockville, and Belleville. Clinics in memory disorder assessment and urinary incontinence assessment are also provided through PCCC.

The 16-bed inpatient Geriatric Rehabilitation service, located at PCCC, offers assessment and geriatric rehabilitation services for elderly patients from Southeastern Ontario. The Division also provides inpatient consultation services at PCCC, Kingston General Hospital, and Hotel Dieu Hospital.
